Parent Permission Form
Parent permission is required by NCTC, for each student participating in the Festival. This includes student actors, crew members, student directors, any student involved with your production. Teachers are asked to facilitate this process, to track the completion of forms, and to ensure all students/parents are in compliance with this rule.
2024 Process Update: Parents will complete a permission form via the website Jotform. Teachers, when you receive your Play Festival confirmation and invoice/receipt, you will also receive two permission form links: 1) a shareable permission form link to distribute to students and parents, and 2) a link to a view-only spreadsheet of submissions on your school-specific permission form to track forms from your students/parents – PLEASE keep this link for your records so you can track who has submitted their form. Links to each school’s permission form will also be available on this webpage.
Teachers can distribute their permission form link via email, or tell parents to find your school’s link on this webpage.
To see the list of parents who have completed the form, use the link to the view-only spreadsheet that you received in your Play Festival confirmation email.
